Please have the following forms completed before you begin the Stormwater Credit Application

Additional Information Required If You Have:
• Rain Garden
• Rain garden design
• Read and completed the Rain Garden Manual for Homeowners
• Rain Garden manual for Homeowners worksheet
• Rain Barrel
• At least 50% of the roof drains into the barrel(s)
• Impervious Surface Removal
• Before AND after photos of the impervious surface
• A surface area of at least 500 sq.ft.
• Pervious Surface
• At least 10 inches of stone
• A surface area of at least 1000 sq.ft.
• Vegetated Filter Strip
• At least 50% of the roof drains into the strip

Quality And Quantity Checklist
Please have the following forms completed before you begin the Stormwater Credit Application

Required Information FOR EACH SCM You Have: Please Check at Least One
• Peak Flow SCM Credit / Site Runoff Volume Reduction / Stormwater Quality SCM
• Site Stormwater Utility Plan / Drainage area maps - Required
• Inspection and Maintenance Plan or Agreement for each SCM - Required
• Legal Agreement for Operation and Maintenance - Required
• Stormwater Management Plan, including design calculations
  - Required, unless documentation from member community is provided
• Documentation from member community ( may include Member Community Review Form - Appendix D)
• Industrial / Marina / No Exposure NPDES Permit
• Proof of Ohio EPA NPDES Industrial Stormwater Permit Coverage ( General or Individual Permit ) - Required
• Applicable Stormwater Pollution Prevention Plan - Required
• Conservation Planning
• Conservation Plan / Forestry Management Plan / Comprehensive Nutrient Management Plan - Required
